What Is Ebola Virus?

Ebola virus disease (EVD) is one of the world’s most severe viral infections. Although outbreaks are relatively uncommon, Ebola can spread rapidly within affected communities and has a high fatality rate if not diagnosed and treated promptly. The disease primarily occurs in parts of Central and Sub-Saharan Africa, where outbreaks continue to challenge public health systems.

The good news is that advances in medical science have significantly improved Ebola diagnosis, supportive care, vaccines for certain Ebola species, and outbreak control measures. Early detection, rapid isolation of infected individuals, and proper medical treatment have greatly increased survival rates compared to previous decades. 

In 2026, Ebola remains an important global health concern due to an ongoing outbreak caused by the Bundibugyo ebolavirus, a less common species for which there is currently no licensed vaccine or approved species-specific treatment. The outbreak has mainly affected the Democratic Republic of the Congo (DRC) and Uganda, prompting the World Health Organization (WHO) to declare it a Public Health Emergency of International Concern (PHEIC). Despite this, the overall risk to the general global population remains low because Ebola spreads through direct contact rather than through the air.

Ebola Virus: Symptoms, Causes, Treatment and Prevention

Ebola virus disease (EVD), previously known as Ebola hemorrhagic fever, is a rare but potentially life-threatening viral illness caused by viruses belonging to the Orthoebolavirus genus. The disease affects both humans and non-human primates, including monkeys and gorillas.

The virus was first identified in 1976 during simultaneous outbreaks in what is now the Democratic Republic of the Congo and Sudan. The disease was named after the Ebola River, located near one of the earliest outbreak sites. 

Scientists have identified several Ebola virus species, including:

  • Zaire ebolavirus (the most deadly and responsible for most major outbreaks)
  • Sudan ebolavirus
  • Bundibugyo ebolavirus
  • Taï Forest ebolavirus

Not every Ebola virus species affects humans in the same way, and available vaccines and treatments may only work against specific species. 

Once the virus enters the body, it infects immune cells and damages the lining of blood vessels, leading to widespread inflammation and vascular leakage. , causing widespread inflammation and damage to multiple organs. In severe cases, it may result in internal bleeding, organ failure, shock, and death. 

The incubation period the time between infection and the appearance of symptoms, typically ranges from 2 to 21 days, with most patients developing symptoms within 4 to 10 days after exposure.

Although Ebola has a reputation for causing widespread bleeding, not every patient develops hemorrhagic symptoms. Many people initially experience flu-like symptoms that gradually worsen if medical care is delayed.

What Are the Causes of Ebola Virus?

Ebola is caused by infection with one of the disease-causing Ebola viruses. Current evidence strongly suggests fruit bats are the natural reservoir. , meaning they can carry the virus without becoming seriously ill themselves. Other wild animals, such as monkeys, chimpanzees, and forest antelopes, may also become infected and transmit the virus to humans.

Human infection generally begins after direct contact with infected wildlife or contaminated animal tissues. Activities that increase the risk include:

  • Hunting wild animals
  • Preparing or consuming infected bushmeat
  • Handling dead wildlife
  • Exposure to infected animal blood or body fluids

Once the virus infects a person, it can spread rapidly through close human contact if proper infection-control measures are not followed. 

Environmental factors such as deforestation, increased human encroachment into wildlife habitats, and climate-related changes may also contribute to the emergence of Ebola outbreaks by increasing contact between humans and infected wildlife. 

How Does Ebola Virus Spread?

Unlike respiratory viruses such as influenza or COVID-19, Ebola does not spread through the air during normal daily activities. Instead, transmission occurs through direct contact with infected body fluids or contaminated materials.

An individual can become infected through contact with:

  • Blood
  • Vomit
  • Saliva
  • Urine
  • Feces
  • Sweat
  • Breast milk
  • Semen
  • Tears
  • Other body fluids of an infected person

The virus may also spread through:

  • Contaminated needles and syringes
  • Medical equipment that has not been properly sterilized
  • Clothing, bedding, or surfaces contaminated with infected body fluids
  • Traditional burial practices involving direct contact with the body of someone who died from Ebola
  • Sexual contact with survivors while the virus remains in semen during recovery 

Healthcare workers are among the highest-risk groups because they frequently come into contact with infected patients. Proper use of personal protective equipment (PPE), strict hygiene protocols, and safe waste disposal significantly reduce this risk. 

People infected with Ebola generally become contagious only after symptoms begin. Individuals who have been exposed but have not yet developed symptoms are not believed to spread the virus.

Is Ebola Airborne?

One of the most common misconceptions is that Ebola spreads through the air like the common cold.

Current scientific evidence shows that Ebola is not naturally transmitted through airborne spread between humans. Casual contact, such as walking past an infected person or sitting in the same room without exposure to body fluids, is not considered a typical route of transmission.

Although experimental studies have explored aerosol transmission under laboratory conditions in animals, there is no evidence that Ebola spreads through the air between humans during normal daily activities. 

Can Ebola Spread Internationally?

Yes, but widespread international transmission is uncommon.

Travelers who are infected may carry the virus to another country during the incubation period before symptoms appear. However, because Ebola requires close physical contact for transmission, health authorities can usually contain imported cases through:

  • Rapid diagnosis
  • Patient isolation
  • Contact tracing
  • Quarantine of exposed individuals
  • Strict infection-control measures

These public health interventions have been highly effective in preventing sustained community transmission outside outbreak regions.

What are the Symptoms of Ebola Virus?

Ebola virus disease (EVD) usually develops suddenly after an incubation period of 2 to 21 days, with most people experiencing symptoms within 4 to 10 days after exposure. During the incubation period, an infected person does not show symptoms and is generally not considered contagious.

The illness often begins with symptoms that resemble common viral infections such as influenza or malaria. Because these early signs are not specific to Ebola, laboratory testing is essential for an accurate diagnosis, especially in areas where Ebola outbreaks are occurring.

Early Warning Signs

The first symptoms of Ebola commonly include:

  • Sudden high fever
  • Severe weakness and fatigue
  • Intense headache
  • Muscle and joint pain
  • Sore throat
  • Chills
  • Loss of appetite

These symptoms may appear mild at first but often worsen rapidly over the following days.

Progressive Symptoms

As the virus spreads throughout the body, more severe symptoms usually develop, including:

  • Nausea and vomiting
  • Severe diarrhea
  • Abdominal pain

Severe Symptoms

In advanced stages, Ebola may cause widespread inflammation and damage to multiple organs, including the liver, kidneys, and blood vessels. Some patients may develop:

  • Bleeding from the nose or gums
  • Blood in vomit or stool
  • Easy bruising
  • Bleeding from needle puncture sites
  • Multi-organ failure
  • Septic shock

It is important to understand that not every person with Ebola experiences bleeding. While hemorrhage is one of the disease’s most recognized features, many confirmed patients never develop obvious bleeding.

Without timely medical treatment, severe dehydration, shock, and organ failure can become life-threatening.

Long-Term Symptoms in Survivors

Many people who recover from Ebola continue to experience health problems for months after the infection. This condition is often referred to as Post-Ebola Syndrome.

Common long-term complications include:

  • Joint and muscle pain
  • Chronic headaches
  • Vision problems
  • Hearing loss
  • Memory and concentration difficulties

Who Is at High Risk?

1. Healthcare Workers

Doctors, nurses, laboratory personnel, ambulance staff, and other healthcare professionals face the greatest occupational risk because they frequently care for infected patients. Without proper personal protective equipment (PPE) and strict infection prevention measures, exposure to infected blood and body fluids can occur.

2. Family Members and Caregivers

Relatives caring for a person with Ebola at home are at high risk if they come into direct contact with the patient’s body fluids without appropriate protection.

3. People Participating in Traditional Burial Practices

Handling or washing the body of a person who died from Ebola can transmit the virus because the body remains infectious after death. Safe and dignified burial practices are a critical component of outbreak control.

4. People Living in Outbreak Areas

Individuals residing in regions experiencing active Ebola transmission have a higher chance of exposure, particularly where healthcare systems are under pressure or contact tracing is limited.

As of 2026, the highest-risk regions remain parts of the Democratic Republic of the Congo (DRC) and Uganda, where the current Bundibugyo ebolavirus outbreak is ongoing. The WHO continues to classify the global risk as low while emphasizing intensive surveillance in affected regions.

5. Hunters and Wildlife Handlers

People who hunt, butcher, prepare, or consume infected wild animals, including fruit bats and non-human primates, have an increased risk of acquiring Ebola through animal-to-human transmission.

6. Laboratory Personnel

Scientists and laboratory staff working with Ebola virus samples require Biosafety Level 4 (BSL-4) containment facilities and specialized safety protocols to prevent accidental exposure.

7. Travelers to Affected Regions

The overall risk for international travelers remains low. However, travelers visiting areas experiencing active outbreaks should:

  • Avoid contact with sick individuals.
  • Avoid handling wild animals or bushmeat.
  • Practice frequent hand hygiene.
  • Follow local public health guidance.
  • Seek immediate medical attention if symptoms develop within 21 days after possible exposure.

Routine travel alone does not place someone at high risk unless there has been direct exposure to infected individuals or contaminated body fluids.

Early Warning Signs and Disease Progression

Ebola progresses through several clinical stages, although the exact course may vary between patients.

Stage 1: Incubation Period (2 to 21 Days)

  • No symptoms
  • Person is generally not contagious
  • Virus multiplies inside the body

Stage 2: Early Illness

  • High fever

Ebola is diagnosed using laboratory tests such as reverse transcription polymerase chain reaction (RT-PCR), antigen detection tests, enzyme-linked immunosorbent assay (ELISA), and blood tests to evaluate organ function. Doctors may also perform differential diagnosis to distinguish Ebola from diseases such as malaria, typhoid fever, dengue, and Lassa fever.

  • Wash hands regularly with soap and water or alcohol-based hand sanitizer.
  • Avoid direct contact with the blood or body fluids of infected individuals.
  • Avoid handling or consuming bushmeat and infected wildlife.
  • Wear appropriate personal protective equipment (PPE) when caring for patients.
  • Follow safe and dignified burial practices.
  • Clean and disinfect contaminated surfaces and medical equipment.
  • Follow local public health guidance during outbreaks.
  • Vaccination is recommended for eligible people at high risk during Zaire ebolavirus outbreaks where approved vaccines are available.
